Health Forms
As discussed on Welcome Night, all new students will need to have the Student Health & Immunization Form completed by a physician. Please make sure you schedule this appointment now, these will need to be completed in the 2024 calendar year. You will upload the completed form to your SchoolAdmin account beginning in April.
If your daughter is considering trying out for ANY sport as a freshmen, please also have the physician complete the required MSHSAA Health Form. Please read the directions for this form carefully, as there are three steps and only step three (page five) will be uploaded to your SchoolAdmin account. Both forms must be completed by a physician in 2024. Please do not email or mail the forms to school. Further directions for submission via SchoolAdmin will be provided this spring.
